Taxpayer Identification Form Number W-9

The Internal Revenue Service requires the Village to maintain the taxpayer identification number and certification for the purpose of reporting certain payments made to persons or companies not classified as employees.

If the federal form W-9 is not on file, the IRS requires the Village to withhold 30% of all payments made to your account.

Submitting the W-9 Form

Village staff strictly follow confidentiality laws and use your tax information for IRS reporting purposes only. To ensure payments can be expedited on your account, please submit a completed W-9 form (PDF) by email to the Finance Department or in person at the Village Hall located at 755 Georgetown Drive, Cary, IL 60013.
